Keeping your child's school informed of health concerns is crucial for assuring a safe and positive school experience. When registering your child for school, completing the online HEALTH section is the first step to ensure your child receives the care that is required. The following information may be helpful as you plan for the school year:

Medication
When a student requires medication during the school day, a parent or guardian must provide the medication in its original prescription container and complete a Student Medication Request Form. A doctor's order is required for all over-the-counter medications. For more information on medication administration, please see the Medication Guideline or contact the school nurse.

Authorization to Carry Medication for Asthma or Anaphylaxis
Per Texas State law, students can carry and self-administer epinephrine and metered-dose inhalers when the necessary authorization form is on file at school. See Permit for Self-Administration of Asthma or Anaphylaxis Medication Form. This authorization form must be renewed each academic year.

Allergies (Life-Threatening)
Parents should inform the district of this vital health information by completing the "Health" section of the online registration. An Allergy-Anaphylaxis Action Plan from your healthcare provider must be on file at school should your child experience an allergic reaction. To learn more about our district's severe allergy and anaphylaxis protocol, including staff and family responsibilities, please contact the school nurse.

Asthma
Parents should inform the district when their child has a history of asthma by completing the "Health" section of the online registration. Providing the school with an Asthma Action Plan from your healthcare professional will help our staff understand and meet your child's needs during the school day.

Seizure Disorder
To ensure that a child with a history of seizures is safe at school, parents must inform the district by completing the "Health" section of the online registration. A Seizure Action Plan from your healthcare provider must be on file at school should your child experience a seizure at school.

Diabetes
For a child with diabetes, a Diabetes Management and Treatment Plan (DMTP) is the first step to ensure your child's health needs are met during the school day. The DMTP is a doctor-provided form for you to review with the school nurse before developing a plan of care for school.
